Introcuction: high risk pregnant women (HRPW) represents 10-20% of all pregnancies and 50% of perinatal mortality. Systemic arterial hypertension (HAS) affects about 10 % of pregnancies and is the leading cause of HRPW. Iodine deficiency disorder (IDD) has important implications for both the mother and the fetus, leading to hypothyroxinaemia, preterm delivery, abortions and psycho-neurological deficit in children. Objective: to Assess if there are nutritional status of iodine differences between pregnant women with and without HAS in a reference center in Salvador, Bahia, Brazil. Methods: cross-sectional study, conducted in a randomly selected 83 HRPW. Questionnaire was designed to obtain socioeconomic, demographic and health information. Urinary iodine concentration (UIC) by Sandell-Kolthoff reaction, and anthropometric evaluation were performed. Results: the overall mean age was 29.4±6.8 years old and median was 30 years old. 73.5%, 24.1% and 2.4% of HRPW were in the third, second and first trimester, respectively. The overall mean UIC was 204±8µg/L and median was 190μg/L (25th-75th percentile: 148-263μg/L, with a range of 2–405μg/L). Low UIC ( 250μg/L, respectively. 39.8% of HRPW had hypertension, and 30.3% of them presented UIC < 150μg/L vs. 24 % in non-hypertensive (OR=1.26; 95% CI:0.62-2.58; p=0.52) Conclusion: it is concerning that 26.5% of HRPW screened at a single center were iodine-deficient.
