: The use of rectal advancement flaps to treat f high anal fistulae is well recognised and associated both cure and preservation of anal continence [1, 2]. The creation of the advancement flap may comprise the rectal mucosa only or involve the full thickness of the rectal wall. Rectal advancement flaps are an accepted approach for the treatmentof complex fistula-in-ano [3]. Although full-thickness rectal advancement flaps offer better results in terms of recurrence than mucosal or partial flaps, all flaps may cause some incontinence. which increases with the thickness of the flap [2].
