Short-Term Results of Reverse Shelf Osteotomy

In acetabuloplasty, the whole grafted bone must adhere to the joint capsule to receive stress loads. When ilium has been chosen as the grafting bone, the cortical bone side has been adherent to the joint capsule. However, contact between the grafted bone and joint capsule might become insufficient if the shapes of the cortical bone and femoral head are not congruent. To improve adaptability between the grafted bone and femoral head, reverse shelf osteotomy allowed adhesion between the cancellous bone side and joint capsule after adapting the shape of the cancellous bone side of the bone graft to the femoral head. Grafted bones in almost all cases showed good remodeling, and clinical results were also very good. Because this new method is minimally invasive and the curative effect appears substantial, this operation should be considered as a potential treatment method for acetabular dysplasia.
