Aim: The aim of this study was to determine the socio-demographic characteristics and psychological and illness conditions of individuals who benefited and / or benefited from HCS conducted within the scope of Canakkale State Hospital between January 2015 and December 2017. Method: In this descriptive epidemiological study, the files of 901 people who were given health services by Canakkale State Hospital Home Health Service Unit between January 2015 and December 2017 were evaluated. A questionnaire was prepared on the information in the files. Sociodemographic characteristics, follow-up periods, bed dependence, personal care, safety, personal hygiene, nutrition, use of auxiliary tools and psychological conditions, chronic diseases, previous surgery, pressure ulcer, neurology, internal medicine consultation, the reasons for the follow-up was recorded. Results: It was found that 62.7% were female, mean age was 77.72 ± 12.78, 40.7% were fully dependent, 26.7% of them were mis-nutrition, 44.9% of them were sad, %13.1 had press ulcer, 7.7% were using auxiliary tools, 99.3% had at least one chronic disease, 22.8% had operation. Conclusion: In order to maintain the physical and psychosocial health status of individuals, there is a need for dieticians, psychologists and psychiatrists and social workers in the service model in primary health care units and in HHS units. HHS should be integrated and coordinated with primary health care services for the success of treatment and the efficiency of this service. The service for people in need of HHS is primarily to create a follow-up model over 65 years of age, followed by primary health care.
