Further reflections on clinical trials in critical care.

At this time when a number of positive trials of ICU interventions are appearing in the medical literature, the authors consider several issues of trial methodology including the impact of 'unblindedness', single versus multi-institutional trials, the problem of 'optimal' cut points and the problem of 'responsiveness' in assessing therapeutic interventions.
