Andropause is a process that involves properly a simple decrease in the testosterone levels so that the proper definition may be a partial androgen deficiency in the aging male. This event may occurs in the early 50 or more in the 60 years of the male and is possible to be detected by a simple questionnaire. However if the testosterone level is in correlation with the symptoms the procedure is easy and therapy is based on strong clinical evidence. Many cases reflect the reduction of muscular mass and increase of body fat, osteopenia or osteoporosis, emotional and cognoscitive disturbances and obviously sexual alteration such as libido reduction and minor quality in the erectile function. It is difficult to have a real diagnosis based on hormonal determination since the low level of total testosterone has not been really determined. If the male has many data in the questionnaire and low but normal testosterone levels the decision of initiating testosterone therapy is still controversial. The medical experience may help to evaluate risks and benefits to help in order to improve the quality of life.
