Surveillance and control of HBV nosocomial infection.

OBJECTIVE To establish a rational method for monitoring measures of hepatitis B virus(HBV) nosocomial infection.METHODS Based on elucidating the relationship of blood volume,infection threshold and virus markers,HBV nosocomial infection were classifyied,then an alternative indicator of source of infection were selected to determine the method for nosocomial infection surveillance.RESULTS The infectious intensity of HBsAg positive and negative,and HBeAg positive and negative patients was 73.7%,9.0%,100.0% and 67.2%,respectively,and their range of infectivity was 0-109ID/ml,0-104ID/ml,102-109ID/ml and 0-106ID/ml,respectively.HBV infection threshold with the blood volume was emerged a negative correlation,and the infectious intensity with the blood volume was shown a positive correlation.When the blood volume was fewer than 10-7ml,only HBeAg positive patients had infectivity.When the blood volume was fewer than 10-4ml,no matter whether carried HBeAg they were,HBsAg carriers possessed infectivity.When the blood volume was more than 10-4ml,all the HBV carriers possessed infectivity.CONCLUSIONS HBV nosocomial infection can be divided into numerous,small and trace amount three types of blood-borne transmission,which can choose respectively HBV DNA,HBsAg and HBeAg alternative indicators of the source of infection,then use the monitoring method accordingly.
