Esansiyel Hipertansiyonda Kalsiyum Kanal Blokeri Nitrendipin’in Kan Basıncına Ve Sol Ventrikül Fonksiyonlarına Etkisi

SUMMARY EFFECT OF NITRENDIPINE ON LEFT VENTRICULAR FUNCTION IN PATIENTS WITH MILD TO (VIODERATE ESSENTIAL HYPERTENSION The effects of 20 mg/day oral Nitrendipine on blood pressure, systolic (ejection fraction (EF), fiber shortening velocity (Vcf), contraction time (Kont.Z. )) and diastolic (relaxation time (Rel.Z.) ) functions of the left ventricle, left ventricular mass (MASS) and posterior wall thickness in mild to moderate hypertension were evaluated. The study was carried out on 21 patients with uncomplicated mild to moderate hypertension for a period of four weeks and 10 normal subjects were taken as controls. Blood pressures were measured in the 1st, 2nd, and 4th weeks, and M-mode echocardiographi c studies were performed before and after the 4th week of treatment. Statistically significant decreases were found in systolic (19 %) and diastolic (17 %) blood pressures. At the end of the study it has been found that Nitrendipine had a effect on systolic, diastolic functions and hypertrophy of left ventricle but it has been concluded that this effect was meaningless statistically. OZET Hafif ve orta dereceli hipertansiyonda orai 4 hafta verilen 20 mg Nitrendipinin kan basinci, kalbin sistolik fonksiyonlari (EF, Vcf, Kont. Z. ), diyastolik fonksiyonlari (Rel.Z.), MASS ve SVADK uzerine etkisi arastirildi. Kontrol grubu olarak 10 normal birey alindi. Calisma grubuna ise hafif ve orta dereceli esansiyel hipertansiyonlu 21 olgu alindi. Tum kontrol grubuna ve 15 hastaya tedavi oncesi ve tedavinin 4. haftasi sonunda ekokardiyografik calisma yapildi. Kan basinclari tedavinin 1., 2. ve 4. haftasi sonunda olculdu. Hipertansif hasta grubunda sistolik kan basinclarinda % 19, diyastolik kan basinclarinda % 17 oraninda istatistiksel olarak anlamli bir azalma gozlendi. Nabizda ise anlamli bir degisiklik olmadi. Yapilan ekokardiyografik calismalarda tedavi oncesi ve sonrasinda anlamli bir farklilik saptanmadi. Meydana gelen yan etkiler gecici olup bir hasta disinda tedaviyi kesmeyi gerektirmedi. Sonucta hafif ve orta dereceli hipertansiyonda Nitrendipinin sol ventrikul hipertrofisi, sol ventrikul sistolik ve diyastolik fonksiyonlari uzerine olan etkisinin istatistiksel acidan anlamli olmadigi sonucuna varildi.
