Prevention in Europe - facts, fallacies and future

Almost 20 years passed since the first decline in CHD mortality in the USA was observed. Since then we constantly hear that not only is the decline continuing, but it is spreading to other countries as well. If CHD is a waning phenomenon, why are we still debating whether and how best prevention should be instituted?
