During the last 10 years ankle replacement has been established as an alternative to ankle fusion. In this review we analyse the problems after total ankle replacement (TAR) in detail and present a review of the literature as well our personal experience about malfunction and failures after TAR. First a summary of possibilties for revisions is given in an algorithm from soft tissue procedures to a complete replacement of both components of the prothesis as well as the explantation and conversion to a fusion. Another problem leading to TAR failure is a flat learning curve and a lack of surgical routine for this challenging procedure, sometimes paired with difficult pathology ranging from malalignments to severe stiffness of the joint.
