Acetabular revision with morsellized allograft and the Harris Galante porous cup. Two to ten year results

2000
Sixty eight cementless acetabular revision were performed using impacted morsellized bone graft and the Harris Galante 1 porous coated component (HGP 1). Acetabular defects were graded according to the American Association of Orthopaedic Surgeons classification. Twenty nine (43%) were contained (Type 2 cavitatory) and thirty nine (57%) uncontained (Types 1 or 3 - segemntal or combined) defects. The patients were followed prospectively for a mean of five years (range 2 to 10 years). Radiological evidence of graft remodelling was seen in 22 hips. Progressive radiolucent lines were present in 8 cases (12%). Two screws broke. There were 2 cases of deep infection. Radiological failure, with a change in cup angle of greater than 5 degress or migration of greater than 5 mm, was seen in 6 cases (9%). All 6 had uncontained defects (segmental or combined). Gratifying medium term results have been obtained after reconstruction of contained acetabular defects using impacted morsellized bone graft and the cementless HGP 1 cup. Inadequate graft support in uncontained defects resulted in 6 cases of failure.
