We thank Dr Tsuda1 for his discussion of possible mechanisms underlying the exercise-induced decrease of blood pressure in resistant hypertension. Aerobic exercise indeed increases the formation of nitric oxide (NO) and improves endothelium-dependent vasodilation in hypertension. Various aspects of endothelial function, such as flow-mediated dilation, endothelial NO synthase expression, or activity of antioxidant enzymes, have been investigated so far. All of them were improved by exercise in previous clinical trials.2 …
