Inferior Oblique Anteriorization

Two common findings in pediatric ophthalmology are inferior oblique overaction and dissociated vertical deviation, often associated with congenital esotropia. Weakening of the inferior oblique is indicated in these situations, accomplished most frequently by a myotomy or anteriorization. The key difference between these two surgical options is an anteriorization not only weakens the muscle, but also changes its force vector, resulting in a more significant decrease in elevation of the eye, helping correct for dissociated vertical deviation more effectively.
