PAPULOPUSTULAR FOLLICULAR ERUPTION CAUSED BY CETUXIMAB – TWO CLINICAL CASES ERUPȚIE PAPULO-PUSTULOASĂ FOLICULARĂ ÎN TIMPUL TRATAMENTULUI CU CETUXIMAB – DOUĂ CAZURI CLINICE

The clinical presentation of cutaneous toxicities associated with anticancer therapies is common and polymorphic. The decision to continue the treatment is made with the interdisciplinary cooperation between oncologist and dermatologist, who take into account the severity of these side effects, particularly their impact on the vital prognosis. We present two patients with papulopustular follicular eruption during treatment with cetuximab, which did not require cessation of this drug. Laboratory findings were normal. The histopathological samples from these two cases confirmed the diagnosis of follicular papulopustular eruption. For the assessment of severity and the effectiveness of treatment we used WoMo Score. After systemic antibiotic therapy with Azithromycin and, Cefixime respectively, combined with application of an anti-inflammatory mixture, the evolution was favorable, without Cetuximab discontinuation. Daily sunscreen was prescribed at hospital discharge for both patients. Follicular papulopustular eruption is a common side effect of molecularly targeted therapy with Cetuximab, which often does not require discontinuation of this treatment. Manifestarile clinice secundare toxicității dermatologice după administrarea de molecule anticanceroase, sunt frecvente și polimorfe. Decizia continuării tratamentului ii aparține atât oncologului cât și medicului dermatolog, care au in vedere gravitatea acestor reacții adverse, indeosebi impactul asupra prognosticului vital. Prezentăm doi bolnavi cu erupție papulo-pustuloasă foliculară in timpul tratamentului cu Cetuximab, care nu au necesitat sistarea acestui medicament. Examenele de laborator au fost in limite normale. Examenul histopatologic a confirmat in ambele cazuri diagnosticul de erupție papulo-pustuloasă foliculară. Pentru cuantificarea severității erupției papulo-pustuloase și pentru urmărirea eficacității tratamentului am folosit Indexul WoMo. In urma tratamentului general cu Azitromicină, respectiv Cefixim, iar local cu o mixtură cu efect antiinflamator, evoluția a fost favorabilă, nefiind necesară intreruperea tratamentului cu Cetuximab. La externare, pacienților li s-a recomandat utilizarea cremelor fotoprotectoare. Erupția papulo-pustuloasă foliculară este o complicație a terapiei  țintite molecular cu Cetuximab, care de cele mai multe ori nu necesită intreruperea acestui tratament.
