Cáncer de piel: Caso columella nasal

Se presenta el caso clinico-quirurgico de paciente femenina de 76 anos conocida por el diagnostico de carcinoma basocelular solido de columella nasal previamente intervenido en el ano 2003. Bajo anestesia general se realizo exeresis amplia de la columella y labio superior el 30-11-2004 con margenes adecuados y negativos desde el punto de vista oncologico. La reconstruccion del labio superior y la columella se llevo a cabo utilizando dos colgajos nasolabiales bilaterales como primer tiempo quirurgico. El segundo tiempo quirurgico se realizo a los 2 meses desinsertando el colgajo nasolabial derecho para construir la nueva y definitiva columella. El resultado de anatomia patologica reporto carcinoma basoescamoso superficialmente infiltrante hasta el musculo estriado. Margen con dermatitis cronica. El resultado clinico-estetico se considera satisfactorio. Skin cancer: Nasal septum case Abstract The authors present to handle of a clinical case of basal and squamous cell carcinoma of the nasal columella in a woman of 76 years old. This case was management in the service of the oncology surgery of the Clinic Nuestra Senora del Pilar, Barinas, Venezuela. The first operation was carried out in the year 2003 by other surgeons. She consults our institution in November 2004. In November 2004 the patient was operated and the lesion was completely removed with oncologic margins. The reconstructive surgery of the high lip and columella was carried out with two bilateral nasolabial healthy skins to cover the wound. After two month, the second time surgery occurred for to separate the right nasolabial graft and to perform the finally columella. The result of the surgical pathologic specimen was reported how: Basal-Cell and Squamous-Cell Carcinoma with superficial infiltration until muscle. The margins were reported with chronic dermatitis. The aesthetic result was considerate satisfactory.
