The reform is realized by the internal and external actors acting together in the process. In this process, neither internal actors nor external actors are decisive in their own right. However, this should not mean that both actor groups act together in the formation of reforms. In some reform implementations, the impact of internal actors is more pronounced, in some cases the impact of external actors is more pronounced. The city hospitals, which emerged as an important reform application in the health system, came to life with the effect of internal and external demands. This study takes this reform application which is very important in health system as a research object. However, in the study, it was not the internal dimension that was effective in the life of city hospitals; focus on the external dimension. The thesis of the study appears at this point and it is claimed that international organizations have a very important position in the birth of the city hospitals. The policy transfer constitutes the conceptual framework of this claim. It is underlined that the policy recommendations by international organizations, especially the World Bank, in the world and Turkey. The United Kingdom practice experience as a result of these recommendations is an example that needs particular attention. Because this example has been inspired in the reform implementation in Turkey. The study also contributes to the public administration literature with this feature and United Kingdom practice, which is not mentioned in academic studies, is also included in the analysis.
