A CASE OF ADULT INTUSSUSCEPTION DUE TO A MALIGNANT LYMPHOMA OF THE ILEUM

This paper presents a recent experience of adult case of intussusception due to a malignant lymphoma of the ileum, together with a review of the Japanese literature. A 28-year-old man visited a nearby hospital because of upper abdominal pain and was pointed out having a cable tumor in the right lower abdomen. The patient was referred to the hospaital. After admission simple X-ray examinatiuon revealed a dilated gas picture in the small intestine. Barium enema disclosed an intussusception ranging from the ileocecal region to ascending colon. CT visualized the intussusception with an ileum tumor 4cm in diameter in the lead. During operation manual reduction was considered impossible and excision of the ileocecal region was carried out. The excised specimen showed that the tumor 4cm in diameter existed in the ileum about 15cm oral-side from the Bauhin vavlve, which led the intussucsception of the ileum and colon. Histopathologically it was malignant lymphoma of diffuse and large cell type. CHOP therapy was added after the operation. Postoperative course has been uneventful.
