Discharge Medications: One Component of a New Inpatient Process

Background: The discharge medication process from a pediatric inpatient unit presents a significant challenge. LEAN analysis of this process at a tertiary, academic hospital revealed key problem areas and pilot changes were directed at those problems identified.
