Ureteroscopic Holmium Laser Lithotripsy for Urethral Calculi: An Analysis of 112 Cases

2013
Objective To explore the clinical effect of ureteroscopic holmium laser lithotripsy on urethral calculi. Methods Clinical data of 112 patients with urethral calculi who were treated with ureteroscopic holmium laser lithotripsy were retrospectively analyzed. Results Among the 112 patients,105(93.7%)achieved successful lithotripsy and 7 underwent failed ureteroscopy. No ureteral injury, perforation and mucosa avulsion occurred in all patients.Operation time was 30-65 minutes and postoperative hospital stay was 4-7 days.Double ureter B ultrasonography and plain abdominal X-ray examination were performed 3-6 weeks after operation and no residual stones were found(stone-free rate,97.3%).Among the 7 patients who underwent failed ureteroscopy,3 were converted to open operation because ureteroscope could not enter the ureteral orifice due prostatic hyperplasia and bladder neck elevation,2 were converted to open operation due to ureteral perforation and blurry vision caused by intraoperative bleeding after stones wrapped with inflammatory granulation tissue were cut and vaporized, and 2 were converted to ureterolithotomy because ureteroscope could not pass through ureters due to obvious tortuous ureter deformity. No complications such as massive hemorrhage, high fever, urine leakage and acute postoperative renal dysfunction occurred in all patients. Conclusion Ureteroscopic holmium laser lithotripsy is a simple, safe and effective treatment and results in fewer postoperative complications in patients with urethral calculi.
