Un caso di schizofrenia resistente

M. is a 45 year old patient with a diagnosis of paranoid schizophrenia, followed by the PSC of Pavia since 1998, when the first psychotic symptoms begin. During these 15 years of disease the patient has undertaken various types of therapy, as none had yielded the desired benefits. After the introduction of quetiapine therapy, titrated up to 1200 mg/day and then suspended for failure to respond after three weeks, we proceed to a drug wash-out followed by a short period of apparent well-being. Due to the reappearance of the symptomatology is set clozapine therapy, with which the patient is still in treatment, despite a not complete therapeutic response. This has therefore led to conclude that the drug therapy in the case of the patient, is essentially a precaution, as the improvements observed during various shelters are almost always presented in a random and independent from psychopharmacotherapy implemented from time to time.
