Taking into consideration the high incidence of uterine fibroids in reproductive aged women and a negative effect on the reproductive potential, one of the main tasks of specialists is to search for effective treatment methods, sparing the reproduction, as well as possible predictors of treatment effectiveness. Aims. To determine the effectiveness of the treatment of uterine fibroids using a progesterone receptor antagonist by examining the effect on the nature of blood flow in the myomatous node in women of reproductive age. Materials and methods. A comparative analysis of the nature of the intratumoral blood flow and the volume of the dominant fibroid in patients of reproductive age after conservative treatment using a progesterone receptor antagonist was performed. The study involved 36 women of reproductive age 20–39 years (average age 31 (4,9) years) with unrealized reproductive plans. All patients received conservative treatment with a drug from the group of progesterone receptor antagonists for 3 months at a dose of 50 mg per day starting on the second day of the menstrual cycle, all patients underwent ultrasound examination of the pelvic organs, Doppler measurement of blood flow in the myomatous node – resistance index (IR), as well as the nature of blood flow, volume of dominant fibroid. Results. In cases of a mixed nature of vascularization, a decrease in the volume of the fibroid was the most significant and amounted to 71% (10,6) of the initial indicators (p <0,01). Conclusion. An echographic assessment of the nature of the blood flow in fibroids can be used as a marker of the effectiveness of the treatment of uterine fibroids using progesterone receptor antagonists in women of reproductive age, thereby optimizing the conservative management tactics and reproductive function.
