Analysis on Characteristics of Lipids and Eyeground Arteriosclerosis in AIDS Patients before Intervention

Objective To explore incidence and characteristics of dyslipidemia and eyeground arteriosclerosis in AIDS patients before anti-AIDS virus treatment. Methods 57 patients without anti-HIV treatment admitted in Beijing Ditan Hospital from June 2009 to November 2011 were selected.Serum lipids,blood glucose and eyeground changes were analyzed. Results(1)Results of serum lipids examination: among the 57 patients,21 patients had abnormal total cholesterol(36.8%),90.5% of which had a lower cholesterol(19/21);20 patients had abnormal triglyceride(35.1%),80.0% of which had increased triglyceride(16/20);51 patients had lower high density level of apolipoprotein(89.5%).Low density level of apolipoprotein was normal in 57 cases.8 patients had abnormal fasting blood sugar(14.0%),of which 6 cases were below standard level and 2 cases were slightly above standard level with abnormal glucose tolerance,but did not reach hyperglycemia level.CD4+ T cell counts varied from 1 to 258 per μl,with an average of(23.69±43.50)per μl and median of 15 per μl.White blood cell abnormalities which were mainly reduction were found in 36 cases among 57 patients,accounting for 63.2%.Erythrocyte reduction was found in 39 cases(68.4%),hemoglobin reduction in 35 cases,accounting for 61.4%,platelet abnormalities in 7 cases,accounting for 12.3%.(2)Results of eyeground examination:among 57 patients,retinal arteriosclerosis level Ⅰwas found in 27 cases(47%),level Ⅱ in 9 cases(16%),level Ⅲ in 2 cases(4%),level Ⅳ in 1 cases(2%).(3)Complications:among 57 patients,40 cases were associated with lung infection(70.2%),35 patients with varying degrees of anemia(61.4%),28 cases with gastrointestinal infection(49.1%),15 cases with intracranial infection(26.3%),14 cases with combined eye infection(24.6%),8 cases with skin and mucosa infection(14.0%),8 patients with hepatitis C(14.0%),5 patients with hepatitis B(8.8%). Conclusion Most HIV-infected patients into the AIDS phase are associated with multi-system and multi-organ infection.Before antiviral intervention,they have already had abnormal lipid metabolism,mainly the low blood high-density lipoprotein.More than half of the patients have varying degrees of retinal arteriosclerosis.HIV infection is related to dyslipidemia and retinal atherosclerosis.
