F-116 Comorbidities associated with health-related quality of life based on COPD severity: The Korea COPD Subgroup Study (KOCOSS) Cohort

COPD patients often have poor health-related quality of life (HRQoL) disproportionate to their grade of airflow limitation. This study evaluated factors responsible for poor HRQoL according to the severity of airflow limitation. Data of 1,310 COPD patients were obtained from KOCCOS cohort. Patients were categorized into two groups; mild-to-moderate and severe-to-very severe COPD patients. We evaluated the clinical factors associated with poor HRQoL in each COPD group. Of 1,310 patients, 946 (72.2%) had mild-to-moderate airflow limitation and 364 (27.8%) had severe-to-very severe airflow limitation. While 59.1% of mild-to-moderate COPD patients had poor HRQoL, 80.2% of severe-to-very severe COPD patients had poor HRQoL. Regarding association between SGRQ-C score and post-BD FEV1 %pred, whereas a very weak association was identified in mild-to-moderate COPD patients(r=-0.103), a weak association was identified in severe-to-very severe COPD patients (r=-0.217). In multiple logistic analysis, a lower education level, higher dyspnea scale, and greater total Charlson comorbidity index with congestive heart failure, hyperlipidemia, and depression are associated with poor HRQoL in mild-to-moderate COPD patients. In comparison, higher dyspnea scale and previous pneumonia history were associated with poor HRQoL in severe-to-very severe COPD patients. Besides lung function impairment and higher dyspnea scale, poor SGRQ scores were affected by extra-pulmonary comorbidities in mild-to-moderate COPD patients and by respiratory-related comorbidity in severe- to-very severe COPD patients.
