ANALYSIS OF PREGNANCY OUTCOME OF ABNORMAL GESTATIONAL GLUCOSE METABOLISM AND GESTATIONAL DIABETES MELLITUS PATIENTS

[Objective] To investigate the impact of the abnormal gestational glucose metabolism and gestational diabetes mellitus on pregnancy outcome.[Methods] 729 pregnant women were received the 50g glucose challenge test(50g GCT).Those whose 50g GCT were abnormal would receive 75g GCT.Based on the result,the abnormal subjects were allocated to gestational diabetes mellitus group(GDM group) and gestational impaired glucose tolerance group(GIGT group),and the others were normal group(NGT group).The pregnancy outcomes of the three groups were analyzed.[Results] In the GDM group,the gestational hypertension,premature rupture of membranes,polyhydramnios,postpartum hemorrhage,and cesarean section were 21.1%,15.8%,10.5%,5.3% and 63.2%,respectively;and in GIGT group,they were 12.5%,12.5%,8.3%,4.2% and 50% respectively.Compared with the NGT group,there were more gestational hypertension,premature rupture of membranes,polyhydramnios and cesarean section cases in GDM group and GIGT group(P﹤0.05).While the premature rupture of membranes and polyhydramnios cases were much fewer in GIGT group than in GDM group(P﹤0.05).As for the neonatal condition,in GDM group,the fetal distress,fetal macrosomia,premature infants,neonatal hyperbilirubinemia and hypoglycemia were 5.3%,21.1%,15.8%,26.3% and 15.8%,respectively,the incidence was much higher than NGT group(P﹤0.05);and in GIGT group,the incidence were 4.7%,12.5%,8.3%,12.5% and 4.2%,respectively,still higher than NGT group(P﹤0.05).But compared with GDM group,there were fewer fetal macrosomia,neonatal hyperbilirubinemia and hypoglycemia in GIGT group(P﹤0.05).[Conclusion] GDM and GIGT would lead to higher incidence of obstetric complication.It's vital to strengthen the perinatal examination to avoid the outcomes led by abnormal gestational glucose metabolism.
