Common variable immunodeficiency (CVID) is a heterogeneous syndrome characterized immunologically by hypogammaglobulinemia and impaired antibody response. It is the most common of the primary immune deficiencies, and clinically characterized by a host of systemic manifestations, including recurrent sinopulmonary infections, autoimmune disorders, and lymphoproliferation. The pulmonary manifestations of CVID may be infectious or noninfectious (Box 1). The infectious complications are more common and must be excluded before a noninfectious, CVID-associated, pulmonary diagnosis is rendered. Examples of infectious complications include
