The preoperative workup of patients “has to make use of all evidence available, has to weigh the significance of each piece of evidence against others, and thus to arrive at the most logically consistent concept of what constitutes the nature of the underlying pathology, whether it is defined structurally, functionally, or both” (Gloor 1987 a).
