A Symptomatic Minor Instability of the Lateral Elbow (SMILE) is a possible etiology of recalcitrant lateral elbow pain: in this condition, the deficiency of the radial band of the lateral collateral ligament as a static elbow stabilizer plays a relevant role, determining hypermobility of the radial head, chondral abrasions, and synovitis associated to intra-articular pain and tendinopathy of the extensor carpi radialis brevis associated to extra-articular pain.
