Currently, the increase in the availability and timeliness of the provision of medical care at the post hospital stage to patients who have undergone acute coronary syndrome, acute cerebrovascular accident, surgery on the heart and great vessels is an acute problem, especially for the health care of St. Petersburg. In St. Petersburg, in comparison with both the all Russian indicators and the indicators of the North Western Federal District (Nwfd) as a whole, it is diseases of the circulatory system (Bsc) that play a leading role in the structure of mortality. The death rate from Bsk in 2019 was 616.7 per 100 thousand of the population, which is 56% of the total mortality structure in St. Petersburg; in the Russian Federation - 573.2 per 100 thousand population (47%); in the North Western Federal District - 642.6 per 100 thousand population (52%). A selective analysis of outpatient patient records in St. Petersburg revealed the absence of dispensary monitoring of patients with circulatory system diseases in more than half of cases (54.6%); a high frequency of inappropriate lipid lowering and antithrombotic therapy; in 43.5% of cases, the absence of prescriptions for obtaining medication. The need for active dynamic monitoring of patients who have suffered a vascular catastrophe makes it urgent to create a service for the prevention of atherothrombosis on the bases of regional vascular centers of hospitals in St. Petersburg. A group of students, cycle profilephotos in the training process, developed a project to create a service for the prevention of atherothrombosis on the basis of the outpatient consultation Department of Spb Gbuz «City hospital № 26», the structure of the service, the necessary resources for its establishment and operation, rules of work and interaction with out patient polyclinic link of health care of St. Petersburg. The organization of the service for the prevention of atherothrombosis on the basis of the outpatient and consultative department of the hospital, which has a regional vascular center in its structure, will ensure continuous monitoring of patients of these categories and in the long term will help to reduce the mortality rates from diseases of the circulatory system and reduce the financial burden on the health system
