Long term prognosis of the coronary artery with negative echo findings in the cases with a history of Kawasaki disease

To clarify the purpose described above, studies were doneas follows. The background of thecases was; 4,398 outpatients with a history of Kawasaki disease, and 1,349 cases with catheterization among them. 1. Histological findings of the coronary arteries, at 14 segments with apparently normal angiographic findings in 4 autopsied cases, were all abnormal. 2.2D echo findings revealed a transient increase of echo density around coronary artery in 100% of 60 cases. 3. In 0.8% outof 258 cases, definite lesions of the coronary artery newly appeared later, even 9 years after the onset. 4. Ultrafast CT revealed calcification atthe aneurysmal wall with partial regression in one out of ten cases. 5. Clinical observations including; ~. limitation of the detection of coronary artery lesion with echo or ECG, ii). high incidence of progressive worsening of coronary artery stenosis, iiO. limitation of diagnosis of myocardial ischemiadue to thestenosis. In our conclusion, long term follow-up of the patients with ahistory of Kawasaki disease is necessary at present, even in the cases with no abnormality on echo, ECG and/or symptoms. 4O
