The Efficacy and Safety Using Typing and Screening Procedure in Elective Case of Orthopedic Patients at Khon Kaen Hospital

The appropriate use of blood during operative procedure was usually required. Because of more request of un-transfused blood caused excessive budget and sometimes lack of blood supply. The routine preparation of blood was Grouping and Cross-matching ( G&M ) which included ABO, Rh system and antibody screening test. And consequence by cross-matching with donor’s blood. This technique was taken time and consumed much money. For the Typing and Screening ( T&S ) technique was performed only ABO, Rh and antibody screening. If the patients required blood during operative procedure, cross-matching will do immediately. The prospectivestudyofassessingtheefficacyandsafetyoftheT&Sprocedurein 628casesofelectiveoperation during January 2006- June 2006 in Orthopedic Department, Khon Kaen Hospital. The request blood were 1,464 units of PRC. There were 48 units of PRC transfused to 34 patients. The transfusion ratio was 5.4%. ( 34/628 ) The cost of G&M technique was 263,520 baths. For the T&S technique was 66,640 baths which lower than G&M 196,880 baths. This study demonstrated that T& S technique should be performed in elective cases which transfusion ratio is quite low. ( < 10% ) No reaction was observed in this procedure and this procedure can be conducted in the Orthopedic Department that should reduce the cost and showed assurance of patients safety.
