Severe toxic keratopathy secondary to topical anesthetic abuse.

Objective: To describe the clinical findings in three cases of abuse of topically administered ophthalmic anesthetics and to review the clinical signs, diagnosis and treatment. Design: Case series. Setting: Two university-affiliated hospitals in Montreal. Patients: Three patients with toxic keratopathy due to abuse of topically applied anesthetics. Results: The three patients presented with a nonhealing epithelial defect, marked stromal edema, folds in Descemet's membrane and a typical stromal ring infiltrate. All three required a conjunctival flap, and two underwent penetrating keratoplasty. The drugs (0.5% tetracaine and 0.5% proparacaine) were easily obtained, at the workplace in two cases and by stealing from the ophthalmologist's examining room in the third case. Conclusions: The unrestricted availability of topically applied ophthalmic anesthetics as over-the-counter medications in Canada must be reevaluated. Objectif: Decrire les observations cliniques dans trois cas d'abus d'anesthesiques ophtalmiques a usage topique et passer en revue les signes cliniques, le diagnostic et le traitement. Conception: Serie de cas. Contexte: Deux centres hospitaliers universitaires de Montreal. Patients: Trois patients atteints de keratopathie toxique due a un abus d'anesthesiques a usage topique. Resultats: Les trois patients ont presente un deficit epithelial qui ne guerissait pas, un important œdeme du stroma, des plis dans la membrane de Descemet et un infiltrat stromal annulaire typique. Tous trois ont du avoir un recouvrement conjonctival et deux d'entre eux, une greffe de cornee. Dans les trois cas, les medicaments (tetracaine a 0,5 % et proparacaine a 0,5 %) ont ete obtenus facilement: deux patients se les sont procures au travail, le troisieme l'a subtilise a l'ophtalmologiste au moment de l'examen. Conclusions: Il y a lieu de reevaluer l'acces sans restriction aux anesthesiques ophtalmiques a usage topique disponibles en vente libre au Canada.
