Computer assisted surgery in total hip arthroplasty started in the early 2000s. It developed from a highly complicated CT-based system to an easier and faster image-free one. Despite literature evidence testifying its reliability, navigation system in total hip arthroplasty has not diffused in clinical practice as expected. The main reasons are: costs, intraoperative complexity, absence of guidelines on correct implant positioning. The perpectives could be: systems related to a computerized digital planning, “smart tools”, pinless devices, development of dedicated patient CT or MRI based instrumentation.
