Results of femoro-distal bypass grafting in the absence of a saphenous vein are not encouraging. Neither has the development of new prosthetic materials nor that of biografts led to a substantial improvement of patency rates. Aiming better short- and long-term patency rates a conical PTFE prosthesis has been prospectively used for distal grafting by adding the theoretical advantages of a modified Miller-cuff technique (“venous boot”) for the distal anastomosis. Preliminary results of the first 26 cases are presented.
