Linezolid for treatment of nosocomial infection in patients with hematological disorders: a clinical analysis of 33 cases.

Objective To evaluate the clinical efficacy and safety of Linezolid injection for treatment of nosocomial infection in patients with hematological disorders.Methods We analysed A retrospective study was conducted of 33 patients who were with hematological disorders and treated on linezolid for nosocomial infection in Department of Hematology,Shanghai Ruijin Hospital,between April 2009 and May 2010.Results Before Linezolid treatment,14 Gram-positive strains (77.8%) were detected in 16 patients,including 4 isolates of staphylococcus(12.1%),3 of VRE(9.1%),2 of E.faecium(6.1%),3 of E.faecalis(9.1%) and 2 of E.avium(6.1%).Linezolid was used as a primary treatment in 4 patients(12.1%) and as the second-line therapy in 29 cases(87.9%).The percentage of bacterial eradication was 86.7%.In all 33 patients,infectious symptoms were relieved after 3-5 days of treatment,with a clinical rat of response at 75.8%.Six patients (18.2%)experienced adverse events,including 1 case of mild vomit,1 of nausea,1 of facial puffiness,1 of blurred vision and 2 of mild diarrhea.Conclusion Linezolid appeared effective for treatment of nosocomial infection among patients with hematological diseases.Some patients were switched to therapy on Linezolid after inefficient or non-responsive use of other antimicrobial agents.Significant myelo-suppression due to Linezolid was not identify among patients who had received chemotherapy for hematological disorders.Linezolid was well tolerated and with confirmed efficacy.
