Effect of Coexisting Autoimmune Disorders on Clinical Outcome in Guillain-Barre syndrome: An Analysis from National Inpatient Database (P1.163)

2018
Objective: The objective of this study was to determine whether coexisting autoimmune disorder impacts clinical outcome in GBS patients. Background: Growing literature has proposed an association between Guillain-Barre syndrome (GBS) and autoimmune disorders. Research related to clinical outcomes in GBS patients with coexisting autoimmune disorder has not been given sufficient attention. The resulting information gap adds further complexity to disease management. Design/Methods: We classified our patients with the diagnosis of GBS into two groups. GBS group (without coexisting autoimmune disorders) and GBS Plus group (with coexisting autoimmune disorders). We used Nationwide Inpatient Survey data files from 2005–2011. We identified patients using ICD-9 diagnosis and procedure codes. We compare the rate of clinical outcomes (In hospital mortality, discharge to home or nursing facilities, length of stay and total charges) between GBS and GBS Plus group. All the in-hospital outcomes were analyzed after adjusting for potential confounders using a multivariate analysis. Results: Of 41,142 patients with GBS, 7653 (18.6%) were in GBS Plus group. After adjusting for confounders, the odds of in-hospital mortality and discharge to nursing facilities was not significantly different among both groups. Length of stay (17 ±53 days vs. 10 ±29 days, p= Conclusions: We found no difference in In-hospital mortality and discharge to nursing facilities in GBS group compared to GBS Plus group. Increase length of stay and hospital charges in GBS Plus group is likely due to additional comorbidities, severity of the disease and cost of multidisciplinary care.
