A CASE OF LEIOMYOBLASTOMA PRESENTING WITH INTRAPERITONEAL MASSIVE BLEEDING

More than 200 cases of leiomyoblastoma of the stomach have been reported in the Japanese literature. Among of them cases presenting with intraperitoneal massive bleeding are very rare. This paper describes a case of leiomyoblastoma with intraperitoneal massive bleeding.A 61-year-old man was admitted to the hospital because of epigastralgia and abdominal distension in June 1990. There was a history of undergoing upper gastrointestinal series, abdominal ultrasonography, abdominal CT, and abdominal augiography at the department of internal medicine about one year before, and he was suspected of having a submucosal tumor of the stomach fed by the left gastric artery. The patient, however, did not hope to be operated on because of lacking in subjective symptoms, and was being followed strictly. On this admission, aggravating anemia and abdominal CT findings indicated a bleeding from the tumor. Emergency operation was performed. Massive bleeding weighted 1, 750 g was found in the abdomen, and a 12×12×4 cm tumor extraluminally growing from anterior wall of the angular notch was observed. The tumor including normal gastric wall was excised. Histopathological diagnosis was leiomyoblastoma of the stomach.
