Plasma calcitonin gene-related peptide,endothelin and atrial natriuretic peptide and clinical significance in patients with chronic obstructive pulmonary disease and pulmonary heart disease

Objective:To explore the action and clinical significance of plasma calcitonin gene\|related peptide (CGRP),endothelin (ET) and atrial natriuretic peptide (ANP) in patients with chronic obstructive pulmonary disease (COPD) and pulmonary heart disease (PHD).Method:Plasma CGRP, ET and ANP were measured by radioimmunoassay at different stages in 21 cases of chronic bronchitis (group A),in 27 cases of chronic bronchitis with obstructive pulmonary emphysema (group B),in 32 cases of chronic obstructive pulmonary emphysema with PHD (group C) and in 24 normal subjects (group D).Meanwhile,artery blood PaO\-2,PaCO\-2 and the first 1 second force expiratory volume possessing the percentage of the predicated value were also measured.Result:1.The plasma CGRP levels in acute exacerbation stage and stable stage of group B and group C were significantly elevated than that of control group ( P 0\^01).CGRP in acute exacerbation stage was higher than that in stable stages in both group B and group C ( P 0\^01).The plasma ET levels of acute exacerbation stages in group A,B,C and stable stages in group B,C were significantly higher than that of control group ( P 0 01).ET in acute exacerbation stages was higher than that in stable stages in both group B and group C ( P 0 01).The plasma ANP levels of group B,C in acute exacerbation stages and of group C in stable stages were higher than that of control group,respectively ( P 0 01).The plasma ANP levels of group B,C in acute exacerbation stages were higher than that of group B,C in stable stages.2.Linear correlation analysis:The plasma CGRP level positively correlated with PaO\-2 ( P 0\^01) and negatively correlation with PaCO\-2,ANP and ET ( P 0 05,0 05 and 0\^01 respectively).The level of plasma ET was significantly positively correlated with ANP and PaCO 2 ( P 0 01,0\^05 respectively) and negatively correlated with PaO\-2,FEV 1%P( P 0 01).The level of plasma ANP was negatively correlated with PaO\-2 ( P 0\^01) and positively correlated with PaCO\-2 ( P 0\^01).Conclusion:The CGRP,ET and ANP participated in the pathophysiology changes and increase in pulmonary pressure in COPD and PHD.They correlated with the severity of the COPD and PHD and their secretion imbalance lead to the deterioration of the process of the disease.
