A drug composition formulated for inhalation, comprising: a surfactant characterized by an affinity for human alveolar / gas interface, said surfactant comprising at least a portion of a pulmonary surfactant polypeptide mammal or a mimic thereof substantially nonimmunogenic in Humans; and, covalently bonded to said agent, an active pulmonary drug that binds to a target bound to an accessible for pulmonary / gas interface extracellular surface or cell, in which the active pulmonary drug is selected from: an inhibitor of elastase, a corticosteroid, a bronchodilator, an antibiotic, a chemotherapeutic agent.
