CLINICAL ASPECTS OF MULTIDRUG-RESISTANT TUBERCULOSIS

Tuberculosisis still a big health problem in the world.Acording to the World Health Organization (WHO) in 2015 there are registered 10,4 millions new cases with tuberculosis and 1,8 millions died from tubrculosis.The resistance toward antituberculotic drugs is health problem that is threat for control of tuberculosis on global level. In 2015 there are estimated 480 000 cases with multidrug -resistant tuberculosis (MDR-TB) und 190000diedfrommultidrug-resistant tuberculosis. Resistant and multidrug-resistant tuberculosis in Macedonia is under control.In theperiod from 2010 to 2016 there are registered only 17 cases with multidrug- resistant tuberculosis.The aim of this study was to present demographic and other specifics of the patients with multi- drug-resistant tuberculosis such as a gender, age groups, employment, presence of contact with tuberculosis patients,localization of the tuberculosis,portion od new and previously treated patients, types of resistance and outcome of the treatment for these patients.Obtained results have been shown graphically in percentages. For this aim 17 registered patients with multidrug- resistant tiberculosis in period between 2010 and 2016 yearwere analyzed. According to the obtained results,the most frequent werethe age groups between 35-54 years (58,8% from the examinated) and most frequently localization was pulmonary tuberculosis (88.4%). Graphic presentation shows that more than 2/3 of analyzed patients with multidrug-resistant tuberculosis are unemployed(82,4%) which confirms the fact that tuberculosis is social disease. According to data for previously treated, about half of the analyzed were new cases, which confirm the fact, that patients were infected from patients who had multidrug - resistant tuberculosis.At 2/3 of the examinated patients the source of the infection was not verified. According tothe results of the treatment, 35,5% from patients with multidrug- resistant tuberculosis were treated, and 17% died because of the severity of the disease and presence of comorbidities. According to the analysis, it was concluded that the multidrug-resistant tuberculosis in R Macedonia is not serious helath problem with small number of registered patients (17 cases ) in the period of 2010-2016 year,however early diagnosis and successful treatment have been essential for the control of tuberculosis in R Macedonia.
