Hacia una ética intercultural para la atención de salud primaria con migrantes: desafíos para profesionales del mundo social

El presente articulo se enmarca en el campo de la investigacion de las profesiones, en especial del mundo social y sus referentes eticos al momento de intervenir con migrantes. El trabajo social, en la implementacion de las politicas sociales, tiene por finalidad (ethos) el bienestar humano y la calidad de vida de las personas. Sin embargo el ejercicio del quehacer profesional se despliega en una constante tension con los requerimientos institucionales, los profesionales y de los propios usuarios. La accion humana asi analizada genera problemas y dilemas eticos. Las consecuencias de esta tension son las dificultades en la implementacion y ejecucion de las politicas sociales justas y de calidad, en forma espacial con migrantes. Los principios de universalidad, responsabilidad y justicia social, se ven vulnerados en la atencion al migrante en la salud primaria. Esta situacion genera problemas, dilemas e inconsistencias eticas y morales profundas en la relacion entre profesionales, usuarios y las normativas institucionales. La manera que han tenido los TS (en general), de resolver estas contradicciones y tensiones es a traves de una etica aplicada instrumental, basada en la eficacia, eficiencia y racionalizacion de los recursos. El presente articulo busca promover una etica “hermeneutica de la conviccion responsable” de caracter intercultural y civico (Cortina 1993); en donde el contexto institucional facilite un dialogo intercultural, mas simetrico y diverso en la implementacion de acciones destinadas a la atencion de salud. En especial en Chile en que los derechos y deberes de los migrantes se ven vulnerados en forma especial en la atencion de la salud primaria. Palabras claves: Migrantes. Salud Primaria. For an intercultural ethics for primary health care to immigrants: challenges for the professionals in the social world Abstract This paper is situated in the field of professions and researches, especially in the social world and its ethics references at the time of intervene with immigrants. The social work worries about the human welfare and the quality of life. However, the professional activity presents a constant tension with the institutional, the professionals and the users’ petitions. By this point of view, the human action generates problems and ethic dilemmas. The consequences of this tension are the difficulties to implement and perform the fair and virtuous social policies, especially with the immigrants. The principle of universalism, responsibility and social justice are violated with respect to the primary health care of immigrant. This situation generates problems, dilemmas and inconsistencies ethics and moral in the relationship among professionals, users and institutional standards. Social workers have been used the instrumental applied ethics, based in the effectiveness, efficiency and rationalization of resources to solve tensions and contradictions. This article seeks to promote an “hermeneutic of the responsible conviction” ethics with civic and intercultural character. By this, the institutional context subserves the intercultural dialogue which is more symmetrical and diverse with the implementation of actions focused on health attention. This proposition has an especially attention in Chile, where the rights and duties of the immigrants are violated mainly in the primary health care. Key words: Immigrant. Primary Health.
