Hypereosinophiles Syndrom mit Endocarditis parietalis fibroplastica Löffler bei Lymphogranulom Hodgkin

Bei einem 49jahrigen Patienten mit hypereosinophilem Syndrom (hochste Leukozytenzahl 125 000/mm3 mit maximal 80,5% Eosinophilen) traten terminal eine Linksinsuffizienz des Herzens, ein schweres zerebrales Zustandsbild und schlieslich mediastinale Lymphome auf. Pathologischanatomisch wurden eine Endocarditis parietalis eosinophilica und Myocarditis eosinophilica mit Innenschichtnekrosen des Myokards bei Thrombose der intramuralen Arterienaste, ein generalisiertes Lymphogranulom Hodgkin und schlieslich eine disseminierte intravasale Koagulopathie mit multiplen Mikrothromben im Gehirn mit Malazien und in anderen Organen festgestellt. Das hypereosinophile Syndrom ist auf das Lymphogranulom Hodgkin zuruckzufuhren; es ist moglicherweise die Ursache der disseminierten Mikrothrombosen und indirekt der Endocarditis parietalis eosinophilica Loffler. Letztere ware demnach als Ausdruck eines paraneoplastischen Syndroms bei Lymphogranulom Hodgkin aufzufassen. Left-heart failure, severe cerebral changes and mediastinal enlargement occurred in the terminal stages of an hypereosinophilic syndrome (maximal leucocyte count 125 000/mm3, maximally 80.5% eosinophils) in a 49-year-old man. The morbid-anatomical diagnosis was: Loffler's endocarditis (fibroplastic parietal endocarditis) and eosinophilic myocarditis with endocardial and myocardial necrosis and thrombosis of the intramural arteries, generalized Hodgkin's disease, disseminated intravascular coagulopathy with microthrombosis in the brain and other organs. The hypereosinophilic syndrome was due to the Hodgkin's disease, it may also have been the cause of the intravascular coagulopathy and, as a result, an indirect result of the Loffler's endocarditis. The latter may thus be interpreted as a paraneoplastic syndrome of Hodgkin's disease.
