Relationship between serum HMGB1 and IL-27 levels, CAT score and COPD with type II respiratory failure

Objective To analyze the relationship between serum high mobility group box 1 (HMGB1) and interleukin-27 (IL-27) levels, chronic obstructive pulmonary disease (COPD) assessment test (CAT) score and COPD with type Ⅱ respiratory failure. Methods 80 patients with COPD and type Ⅱ respiratory failure admitted to Wuxi Second Hospital affiliated to Nanjing Medical University from October 2016 to October 2018 were enrolled as COPD with respiratory failure group.The other 80 patients with COPD in the same period were enrolled as COPD group.And 80 healthy subjects undergoing physical examination were enrolled as control group.The percentage of forced expiratory volume in one second in predicted value (FEV1%pred), FEV1/forced vital capacity (FVC), CAT score, serum levels of HMGB1 and IL-27 in the three groups were measured.The relationship of CAT score, serum levels of HMGB1 and IL-27 with FEV1%pred, FEV1/FVC and prognosis was analyzed. Results FEV1%pred and FEV1/FVC in COPD with respiratory failure group were significantly lower than those in COPD group and control group (all P<0.05). CAT score in COPD with respiratory failure group was higher than that in COPD group (P<0.05). The serum levels of HMGB1 and IL-27 in COPD with respiratory failure group were higher than those in COPD group and control group (all P<0.05). CAT score and serum levels of HMGB1 and IL-27 were negatively correlated with FEV1%pred (r=-0.405, -0.526, -0.513, respectively, all P<0.05), and they were negatively correlated with FEV1/FVC (r=-0.451, -0.498, -0.570, respectively, all P<0.05). CAT score and serum levels of HMGB1 and IL-27 of the dead patients were higher than those of the survivors in COPD with respiratory failure group (t=4.918, 4.175, 6.133, respectively, all P<0.05). Conclusions The serum levels of HMGB1 and IL-27 and CAT score are closely related to the development and prognosis of COPD with type Ⅱ respiratory failure.They are of certain application value for disease monitoring and prognosis evaluation.
