ES EL HOMA UN INSTRUMENTO ADECUADO PARA EL DIAGNÓSTICO DE INSULINO RESISTENCIA EN PACIENTES CON SÍNDROME DE OVARIO POLIQUÍSTICO

RESUMENUn hallazgo fisiopatologico frecuente en pacientes con sindrome de ovario poliquistico (SOP) es lainsulino resistencia, presente hasta en un 70% de estas mujeres. El patron de oro para su diagnostico loconstituye el clamp euglicemico-hiperinsulinemico, sin embargo lo engorroso de su realizacion ha hechopreferir clinicamente el uso del HOMA (Homeostasis Model Assessment) que tiene la ventaja de susimplicidad y bajo costo. Esta revision tiene por objeto verificar la validez del HOMA como instrumento dediagnostico de insulina resistencia en pacientes con SOP. Para esto se realizo una busqueda detallada dela bibliografia medica al respecto en diferentes bases de datos. Aplicando distintos criterios se selecciona-ron finalmente 4 articulos que constituyen la base de esta revision. Fundamentados en la evidencia seconcluye que el HOMA constituye una alternativa valida como metodo diagnostico de insulina resistencia.Su correlacion con el clamp euglicemico hiperinsulinemico varia de acuerdo a la patologia estudiada y alestado nutricional, siendo la asociacion mas fuerte en el subgrupo de pacientes obesas o con sobrepeso.PALABRAS CLAVES: HOMA, insulino resistencia, SOPSUMMARYInsulin resistance is present in 70% of patients with PCOS, thus being a common physiopathologyfinding in women who suffer this condition. The gold standard technique for the diagnosis of this metabolicdisorder is the euglicemic-hiperinsulinemic clamp. However, an alternative method exists, which has theadvantage of being simpler and less expensive: the HOMA (Homeostasis Model Assessment). The purposeof this study is to confirm the validity of HOMA as an approach for the diagnosis of insulin resistance inPCOS. To achieve this, a systematic review was conducted in medical literature, applying different criteria.From the data collected, four articles were selected, which met with the established criteria. Based on theevidence found, HOMA appears to be an accurate, alternative method for the diagnosis of insulin resistance.It correlation with the euglicemic clamp varies according to the different pathologies and nutritional conditionof the patient, with obesity and overweight being the strongest association.KEY WORDS: HOMA, insulin-resistance, PCOS
