Identifying high-risk patients from the emergency department to the home.

The High Risk Discharge Assessment Instrument was developed to guide assessment and intervention as well as identify high-risk patients age 65 and older discharged from the hospital Emergency Department to the home environment. This instrument can be received by home care agencies from a hospital and the indicators used by agencies in admission and discharge assessments. The tested tool, based on research and evidence, can improve many areas of patient care.
