Malacoplakia, which has a predilection for the urinary tract, involves the bladder primarily and the distal ureter secondarily.' In cases of renal transplantation this condition has involved the bladder andlor transplant kidney but never the transplant ureter Although malacoplakia is believed to be less prevalent since the advent of cycl~sporine,~ we report a case of isolated malacoplakia of the transplant ureter manifesting as obstruction, sepsis and anuric renal failure.
