Noticing of an incident before it happens helps to prevent potential damages due to the incident by taking precautions. Therefore, the determination of incidents is significant in terms of reduction of realization possibility of unintended incidents. In this study, it was aimed to evaluate the knowledge level and increase the awareness level of our hospital staff, and to make them report the incidents they had encountered or observed in the last 3 months with the help of a questionnaire. The sampling of the survey was determined to be at least 20% of the professional groups directly involved in the patient care and service processes that have 50 or more staff and the survey was applied to a total of 290 staff. When the survey results were evaluated; it was observed that 96,90 % of the participants knew what the near miss incident was, 91,03 % of the participants defined correctly the near miss incident case related to the safety of patients and 81,72 % of the participants defined correctly the near miss incident case related to the safety of staff. 16, 90% of participants stated that they have observed a near miss incident related to safety of patients or staff during the last 3 months. However, 69, 77 percent of these incidents had not been reported. It was observed that the ratio of the participants who stated that they hadn’t had any time for reporting was 26,66% and who stated that they hadn’t known that it was mandatory to report was 23,33%. As a result, the planning of studies to ensure the reporting of incidents related to both staff and patient safety will contribute to the establishment of safer hospitals for patients and staff.
