Abstract 16889: Ventricular Arrhythmia (VA) in Hypertrophic Cardiomyopathy (HCM): Novel Arrhythmia Risk Stratification Using T1 Mapping, Late Gadolinium Enhancement MRI (LGE-MRI), and Computational Modeling

Introduction: Current measures fail to precisely determine VA risk and implantable cardioverter defibrillator need in HCM patients. Emerging cardiac MRI-based tissue characterization methods and new insights into HCM-specific myocyte properties can be used to create personalized, MRI-based ventricular models of HCM patients that can be used to test arrhythmia propensity. In this pilot study, we developed a novel HCM-specific Virtual-heart Arrhythmia Risk Predictor (HCM-VARP) for noninvasive VA risk stratification. Methods: As a retrospective pilot study, we developed a novel methodology to construct patient specific 3D computational models by combining post-contrast T1 maps and LGE-MRI scans. Two patients with known VA were included. Personalized ventricular geometry was reconstructed from LGE-MRI; fibrotic remodeling was represented in each model by matching a histogram of LGE-MRI intensity to that of T1 time (Fig A).
