Organic disease is responsible for male impotence in more than 50 % of cases. The majority of these cases are due to arterial insufficiency with or without associated venous incompetence. Arteriogenic factors can be diagnosed by doppler ultrasound. Improvements in the contrast media, catheter and guidewire technology and the development of digital arteriography have resulted in penile arteriography becoming a more reliable and much less invasive procedure. Dysfunction of the penile veno-occlusive mechanism is studied by pharmaco-cavernography and cavernometry. Pudendal angioplasty for penile revascularisation remains an experimental technique
