Clinical analysis and pathogenic bacteria of 697 patients with infectious endophthalmitis

Objective To analyse the pathogenic factors, commonly pathogens and prognosis of infectious endophthalmitis. Methods The data of 697 patients with infectious endophthalmitis from Jan. 2013 to Dec. 2017 were analyzed retrospectively. Results The main pathogenic factors of infectious endophthalmitis was trauma(69.15%), followed by eye surgery(17.36%), especially cataract surgery.There were 529 males and 168 females.The patients between the ages of 18 and 65 was the most, accounting for 64.71% (451/697). The positive rate of bacterial culture was 24.76% (129/521), of which the highest positive rate bacterial culture in vitreous was 46.67% (63/135). Staphylococcus epidermidis occupied for 54.26%(70/129), it was sensitive to Vancomycin, Linezolid, Minocycline and Rifampin, and the sensitiy of Fluoroquinolones and Aminoglycosides ranged from 44.29% to 74.29%, low sensitive to Penicillin and Macrolides.The Gram-negative organisms were more sensitive to most other drugs except for Cotrimethoxazole, Minocycline and Ceftriaxone.Streptococcus bacteria have higher resistance rates to Macrolides, Clindamycin and Tetracycline, and more than 80% sensitivity to other drugs. After treatment, 105 eyes (15.06%) were uncontrollable due to infection, and eviseration or enucleation of eyeball were performed, 43.62% (304/697) patients had different degrees of visual recovery after comprehensive treatment. Conclusion Infectious endophthalmitis is mostly caused by ocular trauma and eye surgery, with poor prognosis. The pathogenic bacteria are Staphylococcus epidermidis, Streptococcus pneumoniae and Pseudomonas aeruginosa mainly.
