The invention discloses an intra-light water delivery laser shock peening method and a device. A spray head which is arranged in a hollow laser beam formed after light path transformation of a circular laser beam and at the bottom of a main water pipe coaxial with the hollow laser beam sprays water to a to-be-processed area of a workpiece in the whole circumferential direction to form a stable water restraint layer with uniform thickness, so that the uniform distribution of residual compressive stress on the surface of the workpiece is better ensured. The water restraint layer formed by the method is not influenced by the surface appearance and surface quality of the workpiece, and the method is suitable for processing the curved workpiece, such as a blade. In addition, the adopted hollow laser beam also facilitates significantly reducing a stress hole effect on the surface layer of the workpiece.
